Please read through the following information, it is important for your appointment.
CHIROPRACTIC:
• Families: Please book each patient a unique appointment under their own name
• If it has been over 12 months since your last chiropractic appointment, you will need a Reassessment.
• Dr. Elyse Soleas and Dr. Stephanie Imeson are currently accepting new patients.
• Please take notice of the type of appointment you are booking, they are based on age.
If your visit is due to a motor vehicle accident (MVA) or workplace injury (WCB) please call 780-450-0555 to book.
NEW PATIENTS:
If this is your first visit to our clinic, you will be prompted to fill out forms online through the system. If you’d prefer to fill it out in person that’s fine too! Just please arrive 10-15 minutes early. :)
Please note that we require 24 hours for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee to the card on file. By booking an appointment you are agreeing to this policy.
